Allowances for commune health workers from July 1st

Commune health workers can receive the following allowances:

- Leadership position allowance:

Based on the provisions of Article 5 of Circular No. 06/2024/TT-BYT dated May 16, 2024 of the Ministry of Health, for leadership positions of commune, ward, and town health stations in ranked medical non-business units, they may enjoy leadership position allowances with allowance coefficients of 0.3 (applied to the Head of the Commune, Ward, and Town Health Station) and 0.2 (applied to the Deputy Head of the Commune, Ward, and Town Health Station), the base salary level applied from July 1, 2026. Currently, health station leaders include the Director and Deputy Director.

The level of position allowance for leaders of non-ranked medical non-business units is implemented according to the provisions of Circular No. 83/2005/TT-BNV dated August 10, 2005 of the Ministry of Home Affairs guiding the amendment and supplementation of the salary grade increase regime and salary allowance regimes for cadres, civil servants, and public employees.

- Professional preferential allowance:

Based on the provisions of Point b) Clause 7 Article 6 of Decree No. 204/2004/ND-CP, professional preferential allowances are calculated according to the current salary level plus leadership position allowance and seniority allowance exceeding the frame (if any).

Based on Clause 3, Article 3 of Resolution No. 261/2025/QH15 stipulating that people who regularly and directly practice medical expertise at commune-level health stations and preventive health facilities are entitled to professional preferential allowances as follows:

+ 100% for ethnic minority and mountainous areas, areas with difficult socio-economic conditions, areas with particularly difficult socio-economic conditions, borders, and islands;

+ Minimum 70% for other cases not falling under the above case.

- On-duty allowance:

Based on Point a) Clause 3 Article 3 of Decree No. 192/2026/ND-CP, commune health workers on 24/24 hour duty on working days will receive a duty allowance of 185,000 VND/person/duty session.

If on duty 24/24 hours on weekly holidays, the duty allowance is calculated as 1.3 times the above regulated level; on duty on holidays and Tet, the duty allowance is calculated as 1.8 times the above regulated level. In addition, medical staff participating in 24/24 hour duty may be additionally supported with meal allowances of 40,000 VND/person/duty session.

- Permanent residency allowance:

Based on Point b) Clause 3 Article 3 of Decree No. 192/2026/ND-CP, commune health workers on duty under the permanent residence regime will be entitled to a permanent residence allowance of 9,000 VND/person/session.

- Epidemic subsidy:

Based on Point b) Clause 1, Article 5 of Decree No. 192/2026/ND-CP, in case commune health workers participate in epidemic prevention, they may be entitled to an epidemic prevention allowance from 210,000 VND to 420,000 VND/day/person, depending on the type of infectious disease.

In case commune health workers participate in epidemic prevention on weekly holidays, the epidemic prevention allowance is calculated as 1.3 times the above regulated level. If participating in epidemic prevention on holidays and Tet, the epidemic prevention allowance is calculated as 1.8 times the above regulated level.

- 24/24 hour duty allowance for epidemic prevention:

Based on Clause 2, Article 5 of Decree No. 192/2026/ND-CP, commune health workers when participating in 24/24 hour on-duty epidemic prevention may be entitled to an allowance on weekdays of 280,000 VND/day/person, applied when there is information about an epidemic; on weekly holidays equal to 1.3 times the on-duty allowance on weekdays; on holidays and Tet, equal to 1.8 times the on-duty allowance on weekdays.

Workers participating in 24/24 hour on-duty epidemic prevention are supported with meal allowances of 40,000 VND/person/on-duty session; people directly supervising, investigating, taking, processing disease samples, testing, and verifying epidemics; participating in epidemic prevention are supported with meal allowances of 40,000 VND/person/day.

- Regional allowance:

Based on Clause 3, Article 6 of Decree No. 204/2004/ND-CP and Circular No. 11/2005/TTLT-BNV-BLDTBXH-BTC-UBDT guiding the implementation of regional allowance regimes amended by Circular No. 23/2025/TT-BNV dated December 24, 2025 of the Ministry of Home Affairs, commune health workers working in remote, isolated and poorly climatic areas may be entitled to regional allowances according to a coefficient of 0.1 to 1.0 of the base salary corresponding to the allowance level applied from July 1, 2026, which is from 253,000 VND to 2,530,000 VND, depending on the specific area.

- Special allowance:

Based on the provisions of Decree No. 204/2004/ND-CP and Circular No. 09/20005/TT-BNV of the Ministry of Home Affairs amended and supplemented by Circular No. 24/2025/TT-BNV dated December 24, 2025 of the Ministry of Home Affairs, in cases of working in remote islands and border areas with particularly difficult living conditions, commune health workers may be entitled to special allowances with levels of 30%, 50%, 100% of the salary applied from July 1, 2026 plus leadership position allowance and seniority allowance beyond the frame (if any).

- Attraction allowance:

Based on Clause 5, Article 6 of Decree No. 204/2004/ND-CP and Article 4 of Decree No. 76/2019/ND-CP, commune health workers working in areas with particularly difficult socio-economic conditions for no more than 05 years (60 months) will be entitled to an attraction allowance equal to the salary applied from July 1, 2026 plus leadership position allowance, seniority allowance beyond the frame (if any).

- Allowance for long-term work in areas with difficult socio-economic conditions:

Based on Article 5 of Decree No. 76/2019/ND-CP, cases where commune health workers work for many years in areas with particularly difficult socio-economic conditions may be entitled to allowances with the allowance level calculated according to the base salary.

Specifically, the level is 0.5 corresponding to 1,265,000 VND/month for people with actual working time from 5 years to less than 10 years; the level is 0.7, corresponding to 1,771,000 VND/month for people with actual working time from 10 years to less than 15 years; the level is 1.0 corresponding to 2,530,000 VND/month for people with actual working time from 15 years or more.

- Harmful and dangerous allowance:

Based on Clause 7, Article 6 of Decree No. 204/2004/ND-CP and Section II of Circular No. 07/2005/TT-BNV, in case commune health workers work in hazardous and dangerous environments, they may be entitled to additional allowances from 0.1 to 0.4 of the base salary depending on hazardous and dangerous factors at the workplace, corresponding to the allowance level applied from July 1, 2026, which is 253,000 VND to 2,530,000 VND, depending on the specific area.
